Welcome to Lakeside Oral, Facial and Dental Implant Surgery, and thank you for choosing us. During your first visit, you can expect an oral exam to allow our team to fully diagnose your condition, and an initial consultation with Dr. Bale. Treatment and anesthesia options will be discussed during this time.
On occasion, we can complete your surgery on the same day as your initial consultation. If you have a complex medical history or treatment plan, you will likely require a second appointment to provide treatment on another day.
To get the most out of your first visit, please bring the following with you:
- Referral slip from your dentist
- Any applicable X-rays
- List of current medications
- Medical and/or insurance information
- Driver’s license or other government-issued ID
If you are under 18 years of age, a parent or guardian must accompany you to every visit.
Lakeside Oral, Facial and Dental Implant Surgery wants you to have the best outcome, so please be sure to alert us to any medical conditions you have that may be of concern. This includes diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, and other significant medical conditions.
X-Rays
If you have X-rays from your dentist or physician to share with us, you may request that they forward them to us, or you may pick them up and bring them with you to your first appointment. It is possible that additional imaging may be required to diagnose your condition and execute your treatment.
